If you are unwell with a high temperature (37.8C or higher) or a new continuous cough, please stay at home and self-isolate for 7 days from the onset of your symptoms regardless of your travel history or contact with confirmed cases. PLEASE DO NOT ATTEND THE SURGERY, A PHARMACY OR TO A&E. Please go to the NHS 111 online service if you have serious symptoms that cannot be managed at home or have had no improvement of your symptoms after 7 days. Please only ring NHS111 if you cannot get online. Please note a cough may persist for several weeks for some people. However, this does mean you need to stay at home for more than 7 days.

As you will be aware, COVID-19 virus is a new illness that is spreading across the UK. As such we are taking actions to protect our patients, our staff and our community by introducing a new telephone triage system for GP appointments. Our aim is to significantly reduce the footfall through our surgery in order to reduce the risk of spreading the virus, in line with national guidance.

All GP appointments will initially be on the telephone. If you have an appointment booked with a GP, please do not come to the surgery - please stay at home and the doctor will call you as near to the allocated time as possible. The GP will do all they can to help you over the phone - however, should they feel the need to see you face to face for an examination, you will be invited to the surgery for review. Please either telephone the surgery or use the patient online access service to book appointments. We are no longer booking appointments face to face at reception. Patients will be screened by telephone for all pre-booked nurse and HCA appointments. Nurse and HCA appointments are currently limited to essential appointments only. Please contact reception if you are unsure if your appointment is essential. Please do not come to the surgery unless you have been invited to do so by a clinician.

Due to the COVID-19 virus outbreak and to prevent the spread of infection, for the immediate future we are temporarily disabling the ability for patients to book face to face appointments online. However, you will still be able to book telephone appointments online. Please note you can still request repeat prescriptions online.

Please click here if you have to stay at home because of coronavirus and you need a note for your employer. If you have to stay at home but feel well enough to work, ask your employer if you can work from home. If you can work at home, you will not need an isolation note.

Patient Access to Medical Records on-line

Patient Access to Medical Records online

The practice offers patients age 16 and over the ability to access their medical records online. Patients who are registered to use the practice website can currently request their repeat medications and book or cancel GP appointments on-line.  Patients will now be able to access your medical record online.


Patients already registered for Online Services

 If you are already registered to use the practice website and would like to access your medical records please complete and bring the form below into the Surgery with two forms of ID, one photographic and one recent proof of residence (e.g. utility bill within the past 3 months). We aim to allow access as soon as possible but please understand that individual records need to be examined in case third parties can be identified and that there might be information contained in your record that might cause you distress.

 Patients not already registered for Online Services

If you are not yet registered to use our practice website and would like to be able to book or cancel GP appointments on-line, request medications and have access to your medical record, please complete an application form either at Reception or by completing the form below.  Please bring the form to the practice with two forms of ID, one photographic and one recent proof of residence (e.g. utility bill within the past 3 months) and you will be given login details which will ensure that only you are able to access your record.

It is your responsibility to keep your login details and password safe and secure, as well as any information you print out from your record.  If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.
